Yearly Student Verification
YEARLY STUDENT VERIFICATION
Student verification must be completed EVERY year that your student attends Happy Valley Middle School, whether changes have occurred or not. Through ParentVUE, parents/guardians can complete the verification process and update any changes to their student's information.
Verification and registration for the 2024-2025 school year will begin March 1.
School Bus Procedures
In order to promote safety and efficiency, the school district has set specific regulations for riding the school bus. The driver is the official in charge and that control is absolute. The administration of this school will support the driver without reservation. Violation of bus rules, displays of disrespect toward the driver, other adults, or fellow students will cause loss of bus riding privileges.
Students that need to ride a bus other than the one assigned to them, or get off at a bus stop other than their own, must have signed written permission from their parent/guardian. Phone permission will not be accepted.
Lockers
LOCKERS
Each student will be assigned a locker and issued a combination. Combinations should not be given to anyone other than the occupant of the locker. Lockers are not to be shared. If any locker problem evolves, notify the office immediately. Student lockers are the property of the school and are subject to search. Students are subject to North Clackamas Guidelines for items found in lockers. Periodic locker inspections are made to examine locker occupancy, condition, cleanliness and to check for lost textbook and library books.
School Supply List
As students prepare for school, we are asking students to have the following supplies for classes at Happy Valley Middle School. These supplies should last for the entire school year.
Required School Supplies:

12 pocket Project Organizer w/pencil pouch or Walmart Organizer . This one does not have a pencil pouch, so one will be needed

#2 Pencils - Ticonderoga 12 ct.

4 Color Highlighters (Pink, Yellow, Green, Blue)

5 - 1 Subject Spiral Notebooks (College ruled)

P.E. Uniforms
Students taking P.E. must wear a HVMS P.E. uniform and athletic shoes. P.E. uniforms can be purchased at the Parent Payment portal and picked up in the main office. Uniforms can also be purchased in the main office (cash or check only).

Fee Information
Students are required to pay school fees every semester/year. Payments can be made in the Parent Payment Portal or in the main office (cash or checks only). Make checks payable to HVMS.

Lunches
LUNCHES
We are pleased to announce that starting in September 2024, all North Clackamas School
District students who attend a school that serves breakfast and lunch may receive
breakfast and lunch at no cost. This amazing news is thanks to the expansion
of the National School Lunch Program's Community Eligibility Provisions (CEP) in
Oregon.
What does this mean for you and your children?
● Families will no longer need to complete an application to receive free or
reduced-price meals.
● Middle and high schools will only collect meal money for a la carte
purchases not part of a full breakfast or lunch purchase or for purchases of
a second meal.
From 9:00 a.m.-9:30 a.m. there is an assistant located at one of the lunch computers in the Nest to receive deposits from students. Deposits need to be made by 9:30 a.m. Lunch money can only be accepted in the Nest, not the main office. Make checks payable to NCSD #12
To deposit lunch money ONLINE, parents can set up lunch accounts in LINQ Connect.
Health Room: Sports Physicals & School Medication Information
Health Room
If students are feeling unwell, they are able to come to the Health Room by Student Services where they may call a parent/guardian to pick them up. The student must go home or return to class, they cannot stay an extended period of time in the Health Room. Students with temperatures above 100.4 degrees with be sent home.
Do not send your student to school if,
- they have had a fever within the last 24 hours or,
- vomiting within the last 48 hours.
Student Medication Information
Students may not take or carry any prescription or over-the-counter medications while at school unless written request from the parent/guardian or health care provider is approved. Medication (prescription or over-the-counter) that is needed for your student to remain in school must come in a fully labeled original container. A parent/guardian must fill out and sign the Medication Assistance Form and submit it to the main office.
Only small containers can be kept at school due to limited space. All medications will be stored in the office in a locked cabinet. Parents/guardians will be asked to pick up medications at the end of the year.
Sports Physicals
Sports physicals are required prior to participation in interscholastic sports. Physicals are valid for 24 months from the date it was issued by a health-care provider.
Immunizations
Immunizations
If your student is coming from a North Clackamas School District school, the health and immunization records will automatically be transferred - no need to fill out more forms. A Tdap (Tetanus/Diphtheria/Pertussis) booster given after age 10 is required for 7th grade. If we do not have this in our records, you will get reminders to update immunizations so your student can attend school. For free/low cost immunizations, call the Health Department at (503) 655-8471.
